Paid as an annual fee; cannot be split into monthly payments. Renewal date is the anniversary of your enrollment. Benefits are not transferable and have no cash value.

May not be combined with dental insurance or other discount plans. Honored only at Kingston Bay Dentistry. This plan is not dental insurance and does not supplement existing medical or dental insurance benefits. Exclusions: orthodontics, Botox, whitening, workers’ compensation claims, and specialty providers.
